If the music was to loud, she was distracted. She can't hear whats going in the surroundings.
Driving isn't all about visually seeing everything that is happening in front of you, you need to hear as well. In this case, maybe hearing could have saved a life.

where in there did i say it was her fault? i simply said "An accident could have been prevented". where is that putting the blame on her? It isn't.
